We know things are as far away as ‘business as usual’ and it has been a difficult few months.
We are doing what we can to support and we are asking for your help. We want you to tell us how you are and how the pandemic is affecting you, which of our services are working well and what needs improving.
Your answers will be used anonymously to support our funding applications and develop our services.
Naturally we hope feedback will be good, but we ask you for your honest feedback.
There may seem like a lot of questions but most of them are check boxes and you may find some do not apply to you. Please leave blank any you do not wish to answer. There are a lot of ‘open-ended’ questions as we want to hear from you.
If you prefer a paper form please let us know and we will send one out.
